- Consent for Treatment – required for someone other than a parent, grandparent, adult sibling, or adult aunt/uncle to bring the child to an appointment.
Sports
This form is used by Texas public schools for athletics and marching band participation. Private schools, Scouts, and non-school sports leagues may require a different form. Schools may also require additional forms to be completed by the parent and/or student.
ADHD Evaluation Forms
Completed Parent and Teacher Initial Forms are required to schedule a new ADHD consult visit. If your child has multiple teachers, please give one each to your child’s math teacher, reading/ELA teacher, and at least other academic subject teacher.
