If your child currently has a UHC Medicaid plan, please call 1-800-964-2777 to request to change to a different Medicaid network. Existing UHC Medicaid patients will continue to be seen based on Medicaid benefits until Summer 2024.

Newborns assigned to Wellpoint Medicaid at birth will be seen based on Medicaid benefits until 4 months old. Parents should call 1-800-964-2777 to request to change to a different Medicaid network as soon as possible. This process takes 90 days for newborns.
If your child has an out-of-network commercial insurance, you will responsible for out-of-network charges, which are not eligible for advanced payment discounts. If your child has an out-of-network Medicaid plan, you must change your plan by calling 1-800-964-2777. We are out-of-network with Medicaid plans not listed here, that serve other regions of Texas. Existing patients must have a reference number or verification of plan change before being seen.
Additional Information for Commercial and Marketplace Insurances
If you have applied for insurance for your child, but have not received any updates in 2 weeks or more, please call the insurance company or your employer’s human resources/benefits department to ask about your application status. Sometimes things get lost in the mail.
If you plan to add your newborn to your insurance plan or apply for an individual policy, please do so as soon as possible. Most plans allow a maximum of 30 days after a baby is born to add the baby to insurance. If you miss this window, your baby may not be eligible for insurance until January of the following year.
“HMO”-type insurances, including most Marketplace insurances, require a “Primary Care Provider” (PCP) to be designated on each person’s plan. Please make sure to set Dr. Chang as your child’s PCP before your visit. If Dr. Chang is not the PCP on your plan, you may not receive in-network benefits, and Dr. Chang will not be able to submit referrals or prior authorizations for your child. To assist the insurance representative in identifying Dr. Chang, you may need to provide his NPI (National Provider Identifier) number: 1649405523.
If your child is on a work-sponsored health insurance plan, and the primary policyholder (the person whose job offers the insurance) is no longer employed, please let us know before your visit. It is your responsibility to understand any COBRA benefits.
Insurance companies may “retroactively terminate” insurance policies. Most commonly this is due to a change or end of policy due to change in employment which does not get updated to the insurance company until months later. This may result in many hundred or thousands of dollars of charges becoming “patient responsibility” due to the use of privately-supplied vaccines. This can be avoided by letting us know of any changes before your visit.
It is your responsibility to understand your insurance policy and to promptly inform us of any changes to your policy.

Texas Medicaid is administered through private insurances companies (“MCOs”). These companies set up regional insurance networks around Texas. If your child has Medicaid, and you recently moved into or out of the Houston area [Medicaid Service Area Map], you may need to change your Medicaid plan.
Texas Medicaid plans, except Traditional Medicaid, designate a “Primary Care Provider” (PCP) on each child’s plan. Please call your child’s Medicaid member benefits number to change the PCP to Dr. Chang before your visit. Your child’s PCP will be randomly assigned when first getting onto Medicaid, and may be randomly reassigned after any Medicaid renewal or interruption in coverage.
It is your responsibility to stay up to date on your child’s Medicaid policy. Please make sure the Medicaid plan has accurate phone and address information to contact you.
Additional Information for Self-Pay
Cash pay rates may be allowed a discount on our normal charges for payment-in-full prior to service. Rates are available in office, and start at $75 for established patients.
Children 18 years and under who do not have insurance are eligible for most vaccines under the Texas Vaccines for Children (VFC) program. Vaccines are $13 each.
